Job Summary:

The Ambulatory Registered Nurse (RN) Case Manager provides comprehensive, patient-centered nursing care in an outpatient clinic setting, supporting individuals and families across the continuum of care. This role involves assessing, planning, implementing, and evaluating care; administering medications and treatments; performing clinical procedures; and coordinating care in collaboration with providers and interdisciplinary teams. The RN ensures accurate documentation, adherence to infection control and regulatory standards, and efficient clinic operations while promoting quality outcomes, patient safety, and culturally responsive care in a fast-paced ambulatory environment. An active Alaska RN license and recent case management experience are required.

Responsibilities:

• Perform comprehensive patient assessments and develop individualized care plans.
• Implement and evaluate nursing interventions and patient responses.
• Administer medications and treatments safely and accurately according to provider orders and policies.
• Recognize changes in patient condition and initiate appropriate interventions or escalate care as needed.
• Provide patient and family education related to care, medications, and discharge planning.
• Deliver culturally responsive, patient-centered care.
• Collaborate with providers and interdisciplinary team members to coordinate care and facilitate referrals and follow-up.
• Document assessments, interventions, and outcomes accurately and timely in the electronic health record.
• Advocate for patient care and assist patients in navigating the healthcare system.
• Adhere to regulatory, safety, and infection prevention requirements.
• Participate in quality and performance improvement activities.
• Maintain required competencies and certifications.
• Report adverse events, near misses, and unsafe conditions per policy.
• Provide guidance and delegation to ancillary staff and support orientation and mentoring of staff, students, and peers.
• Float to other departments or assignments as needed.

Required Skills & Certifications:

• Current full, unrestricted Registered Nursing license in Alaska or other U.S. state (active Alaska license required for agency staff; direct hires must have applied before start date).
• Graduate of an accredited school of nursing.
• Basic Life Support (BLS) certification.
• Case management experience in a clinic or hospital setting within the last 2 years.
• High school diploma or equivalent.

Preferred Skills & Certifications:

• Bachelor's degree in Nursing or related field.
• Case Management certification.

Special Considerations:

• Must have active Alaska nursing license at time of submittal.
• Position requires adherence to Alaska Board of Nursing scope of practice and nursing judgment standards.
• Role involves working in a fast-paced ambulatory outpatient clinic environment.
• Traveler start date is contingent on fingerprint card processing; approximately 6 weeks after receipt at SEARHC.
• Agency does not provide housing or transportation; employee responsible for these needs.

Scheduling:

• Work schedule and shifts are based on ambulatory clinic operations; specifics to be determined by employer.
• May involve floating to other departments or assignments as needed.
